Preparing for Your First Appointment with Kitsap Podiatry

Please bring the following with you for your first appointment:

• Insurance cards and co-payment. We work with major insurance carriers and accept credit card payments.
• Any lower extremity x-rays, MRI’s, or CT scans done within the past year.
• The shoes you wear most for work or sports, such as boots, running shoes, etc.
• The registration and privacy/financial agreement forms, if you haven’t already sent them to us.
